Fill Out Your Forms

After scheduling an appointment, you’ll receive an invitation to join our online patient portal, onpatient. This portal will allow you to fill out or update your information, medical history, and reasons for the visit and sign necessary consent forms–all from the comfort of your home. Completing paperwork prior to the appointment will greatly expedite your visit.

What to Bring

Make sure you bring with you the required forms and documents:

  • Your insurance card
  • Your photo ID
  • A referral from primary care physician (if HMO)
  • All relevant medical or laboratory results (X-rays, MRI report, previous doctor notes, etc)
  • Method of payment

We accept cash, checks (not permitted if self-pay), Visa, Mastercard, Discover, Amex, ApplePay, and CareCredit.

When to Arrive

At Modern Foot & Ankle, we know how valuable your time is. For this reason we recommend that all patients complete the check-in process at home prior to their arrival; this greatly reduces any waiting time. If you have completed the check-in process at home prior to your visit, we recommend that you arrive about 5 minutes prior to your scheduled appointment. You may complete all required forms and documents via onpatient by clicking the button below.

If you are not able to complete these check-in forms at home prior to your appointment, we recommend that new patients come to our office 20 minutes before the scheduled appointment time. This will allow time to fill out all required medical forms and documents without delaying your visit.

When To Pay:

At Check-in: Copays and any outstanding bills will be due at check-in prior to seeing the doctor. Payment for office visits is required at check-in for our self-pay patients (checks not permitted).

At Check-out: Payment for cash products and services are due at time of dispensing. Self-pay patients may be required to pay for any additional procedures or services at time of check-out.

We accept cash, checks (not permitted if self-pay), Visa, Mastercard, Discover, Amex, ApplePay, and CareCredit.

Medical Questions: If you have a medical emergency, call 911 immediately. For urgent medical questions that are foot and/or ankle related, you may reach Dr. Siegel directly via direct messaging him through our patient portal, onpatient. onpatient is our preferred method of communicating with our established patients.

How To Pay

Most claims are processed within 30-60 days after your appointment. You will receive an Explanation of Benefits (EOB) from your insurance company along with a patient statement from our office. Patient statements are mailed and sent to your onpatient account. You may pay us via the onpatient portal (credits and debit card payments only) [see below], in person (cash, check, credit, CareCredit), or over the telephone (debit, credit, and CareCredit). We understand that price transparency is vital in containing costs and choose to accommodate patients who are self-insured, or are seeking to lower expenses on non-covered services, by offering a prompt-pay discount; to that end, we’ve listed the cash prices for our common procedures, services, and products below. For anything not listed, please ask a member of our friendly support team for a quote.

Office Visits

  • New Patient Office Visit
    $140

    New patient office visit. Includes history, physical examination, and diagnosis. Does not include any procedures, medical equipment, or x-rays.

  • Follow-Up Office Visit
    $85

    Office visit for each returning visit. Includes follow-up history, physical, and diagnosis. Does not include any procedures, medical equipment, xrays.

X-ray Imaging

  • Ankle X-ray
    $45

    Ankle X-ray Imaging which includes 3 X-ray views of one ankle and physician reading of x-ray. Price does not include office visit, additional procedures, or additional x-ray imaging of other limb or body part.

  • Foot X-ray
    $35

    Foot x-ray imaging which includes 3 X-ray views of one foot and physician reading of x-ray. Price does not include office visit, additional procedures, or additional x-ray imaging of other limb or body part.

In-Office Procedures

  • Routine Nail and Callus Trimming
    $50
    For most insurances, routine foot care (which includes toenail and callus trimming) is not a covered benefit. Understanding the importance of these services to our community, we offer routine foot care for a nominal fee. This price includes trimming and mechanical smoothing of all toenails along with debridement (trimming) of all calluses on the feet. No additional office visit is charged if you are only seen for routine foot care. Any additional foot complaints will require an office visit charge. X-rays and additional procedures are not included. This price is per routine foot care session and there is no limit to number of sessions.
  • Ingrown Toenail, Infected
    $250

    This procedure involves the removal of an infected ingrown nail or nail border, with local anesthesia. This price includes the removal of toenail, injectables, drainage of any infection, along with postoperative dressing. Includes physician technical fee and surgical  supply pack (supplies used by doctor during procedure) fee. Price does not include office visit, x-rays, or any additional procedures. Price does not include any home wound care supplies.

  • Ingrown Toenail, Permanent Removal
    $250

    Removal of chronically non-infected ingrown toenail (either nail sliver or entire nail). This price includes removal of toenail, injectables, medicine to inhibit future ingrown nails, and postoperative surgical dressing. Price includes physician technical fee and surgical  supply pack (supplies used by doctor during procedure) fee. Price does not include office visit, x-rays, any additional procedures, or subsequent office visits. Price does not include any home wound care supplies.

  • Wart Therapy
    $130

    Plantar wart therapy. Price includes thorough debridement (trimming) of wart, application of prescription medication, and surgical dressing. Price does not include office visit, x-rays, other procedures, or subsequent office visits.

  • Steroid Injection (medium-sized joint, tendon, ligament)
    $100

    Medium-sized joint, ligamentous (plantar fascial), or tendinous steroid injection for the purpose of treating inflammation. Price includes injection of steroid into one area of foot or ankle. Price includes medication and supply costs. Price does not include office visit, x-ray, other procedures, or subsequent office visits. Common conditions for this type of therapy includes: big toe joint pain, heel pain (plantar fasciitis), midfoot pain to name a few.

  • Steroid Injection (small-sized joint, forefoot neuroma, hypertrophic scar)
    $85

    Small-sized joint, neuroma, hypertrophic scar steroid injection. Price includes injection of steroid into one area of foot or ankle. Price includes medication and supply costs. Price does not include office visit, x-ray, other procedures, or subsequent office visits. Common conditions for this type of therapy includes: lesser toe joint pain, forefoot neuroma pain, painful hypertrophic (keloid) scar to name a few.

  • Cast, Below-The-Knee Fiberglass Cast
    $90

    Below-the-knee fiberglass cast. Price includes application of fiberglass cast to extremity, supply costs, physician technical fee. Price does not include office visit, x-rays, additional procedures that may be needed. Casts are often needed for severe fractures (broken bones) of the lower extremity.

  • Splint (Lower Extremity Soft Multilayer Splint)
    $80

    Soft multilayer splint, any size, to lower extremity. Price includes application of multilayer splint to extremity, supply costs, physician technical fee. Price does not include office visit, x-rays, additional procedures that may be needed. Casts are often needed for mild to moderate fractures fractures (broken bones), sprains, sports injuries, or soft tissue injuries of the lower extremity.

Ancillary Services

  • Custom-Molded Orthotics
    $450

    Prescription custom-molded orthotics. Includes the cost of one pair of custom-molded orthotics, as prescribed by the physician. Cost includes any subsequent adjustments that may be needed to ensure proper fit (up to five adjustments). Price does not include any applicable office visits, x-rays, casting procedure costs, casting supply costs, or subsequent office visits. Cost of orthotic is due at time of casting.

  • Each Additional Pair of Custom-Molded Orthotics
    $200

    Each additional pair of custom-molded orthotics. If casted within 6 months, additional orthotics are discounted significantly. Price includes the cost of a pair of custom-molded orthotics. Price does not include office visits, x-rays, additional procedures, or subsequent office visits.

  • Amniotic Membrane Injection (with walking boot included)
    $650

    Some of the newest technology on the market, amniotic membrane injection is a minimally invasive injection product that contains growth factors that help your own cells regenerate the damaged tissue, reduce scar tissue formation, and control inflammation. This price includes the cost of the medication, the cost of the supplies (including walking boot), and the physician technical fee. This price does not include any office visit, x-ray, additional procedures, or subsequent office visits. Durable medical equipment, such as a walking boot, will also need to be dispensed and is included in this price. Common conditions for this therapy includes: plantar fasciitis, tendonitis, tendon tear, and arthritic pain to name a few

  • Wiivv Custom Fit Insoles- Full Length Option
    $89

    Modern Foot & Ankle is the first retailer in all of Florida to carry Wiivv custom-fit orthotics. Less custom that prescription orthotics, Wiivv custom-fit orthotics provide a cheaper alternative for relief of heel pain and general foot pain. Price includes the cost of the Wiivv orthotics, along with the 3D measuring of each foot in the office. The price does not include tax, any office visit, x-ray, additional procedures, or subsequent office visits. Adjustments are not allowed for this product, however, Wiivv does have a 30-day return window.

  • Wiivv Custom Fit Insoles- 3/4 Length Option
    $69

    Less custom that prescription orthotics, Wiivv custom-fit orthotics provide a cheaper alternative for relief of heel pain and general foot pain. Price includes the cost of the Wiivv orthotics, along with the 3D measuring of each foot in the office. The price does not include tax, any office visit, x-ray, additional procedures, or subsequent office visits. Adjustments are not allowed for this product, however, Wiivv does have a 30-day return window.

  • Plantar Fasciitis Care Pack
    $40
    Hand picked by Dr. Siegel, the items in this care pack are made specifically for those patients with mild to moderate heel pain. This pack consists of all the items you need for at home exercises to cure your heel pain. Separately, these items cost over $50 which means a savings of at least $15 and a trip to the store.
  • Nail Surgery Care Pack
    This care pack was developed specifically for our post surgical nail patients. It consists of all the at-home dressing supplies and medications you will need for the entire postoperative period.
  • Post Op Bunion Care Pack
    $50
    From bandage protectors for the shower, to adhesive remover, this care package contains eight different items for all aspects of the postoperative period following bunion surgery. This pack takes away the stress of finding each individual item on your own. Best of all, you save money: each item sold separately is $25 over the care pack price.
